How these numbers are calculated
- Registered — patients with hypertension diagnosis (Diagnosis value = 4). Codes: 1=Asthma, 2=Diabetes, 3=Heart failure, 4=Hypertension.
- Under care — registered patients with ≥1 visit in the past 12 months.
- BP controlled % — among patients with a visit in the past 3 months and a recorded BP; systolic <140 and diastolic <90 at latest reading.
- 12-month LTFU — registered − under care (no visit in 12 months).
- Monthly control trend — among BP readings recorded that month, share controlled vs not.
