About this dashboard

Background

Hearts360 is a hypertension and diabetes programme dashboard used by public health teams to monitor registration, retention, control, and overdue follow-up across districts and facilities.

The indicator model draws on WHO HEARTS guidance and practices shared by Resolve to Save Lives and the Simple.org community.

Indicator definitions

Definitions follow WHO HEARTS programme indicators for registration, under care, blood pressure / glucose control, and lost to follow-up.

Hover tooltips and section notes on each dashboard describe numerators and denominators used in MediSoft Hearts360.
